Sperm Count for IUI Treatment – Intrauterine Insemination (IUI) Success Rates

When a patient has mild or minimal endometriosis, unexplained infertility, and a mild male factor, intrauterine insemination (IUI) is a reasonable initial approach. IUI and natural cycle sexual activity both had a similar pregnancy rate. In order to do this, we typically combine IUI with fertility medications (clomiphene citrate, letrozole or gonadotropins).

Sperm Count for IUI Treatment

The Sperm Count for IUI Treatment and seminal plasma are separated from the semen sample during intrauterine insemination by “washing” it in a centrifuge (which contains prostaglandins that can cause severe cramping). The motile sperm are then placed in a catheter and placed in the uterine cavity when the lady is ovulating. The total motile sperm count (TMSC) of the sample is typically estimated before IUI. In general, a post-wash TMSC of more than 10 million is regarded as ideal. Since TMSCs have very poor pregnancy rates, IUI is typically not recommended for these patients. 

They looked at all IUI cycles with stimulated clomiphene citrate, letrozole, and/or injectable gonadotropin that were carried out between 2004 and 2014, omitting double insemination. Insemination cycles totaling 47,553 were available to assess the connection between TMSC and clinical pregnancy (defined as ultrasound confirmation of an intrauterine gestational sac). Pregnancy rates were highest, and a definite threshold of more than 9 million TMSC was identified. As TMSC fell, pregnancy rates eventually reduced (see Table).

Patients having a TMSC higher than 9 million were those with the highest IUI pregnancy rates. Even between the TMSC values of 8 and 9.9 showed a statistically significant decline (p=0.024).
